Student Loan Repayment

I have $25,000 in student loan debt thru Navient(formerly Sallie Mae). It's from the years of 1996-2000. I had a job as a police officer right out of college and paid my loan back religiously. But then I got pregnant with twins who needed special care and attention and quit my job to stay home with them. That was in 2004. My loan has been in forbearance and deferment for years after that until both of those options were unable to get anymore. Now I need to start repaying this loan. I've tried a lower payment but with all the medical bills and out of pocket money we need for medications i can't make the payment. I can't qualify for the income based repayment plan because my husband claims me on our W-2 forms and Navient considers his income in order to come up with a payment. Which isn't fair cause this debt accumulated way before him. I want to pay my loan, I really do but the health of my children come first and I'm struggling now just to make my house payments on top of my children's health care expenses. Is there anything I can qualify for? Anything I can do? Navient calls everyday and the people I talk to don't help at all. I'm desperate to resolve this. Please, can you give me any advice?
